Price for the examination and completion
of Form I-693.

Ages 0-1: $350

Ages 2-17: $550

Ages 18-24: $650

Ages 25-44: $600

Ages 45+: $550

The prices above include required laboratory studies per USCIS if obtained at Deeper Roots Health. Prices subject to change.

Vaccinations are an additional cost if needed, and are available at other facilities, such as our local county Health Departments or Pharmacies. Deeper Roots Health does not provide vaccination services.

Chest Xray if required will be $100 at the office on-site. This can be performed at a different facility but I will need to personally see the xray image. This will incur its own cost and additional radiology fee based on where you have it performed.

Make sure you have the most recent Form I-693. Please print and fill our your portion prior to your upcoming appointment if possible. If unable, we will provide the form for you. After scheduling with us, you will receive an email with information to help complete this form.
