Monthly Membership Pricing

  • Age 0–18 (with 2+ children or adult membership): $25 per month

  • Age 0–18 (without adult membership): $50 per month

  • Age 19–44: $50 per month

  • Age 45–64: $75 per month

  • Age 65+: $100 per month

  • Important for those on MEDICARE -

    • Membership for Medicare will start April 1, 2026.

  • Family Membership Cap: $200/month
    *Family include adult member + spouse/significant other, immediate children under age 21

  • Sliding Scale Fee (Hardship) - Application required. Contact us for more information.

  • Enrollment Fee: $50 per family *waived until July 2026

  • Re-Enrollment Fee of $50 per person will be added to those who leave the practice desiring to re-establish
